My trigger doesn’t work as avertised. Am I doing something wrong?

In the vast majority of cases the answer is YES, the user has done something wrong and it takes some well placed questions on our part and your considered answers for us to figure out what’s wrong. Normally, if we had the lower in front of us, we could see immediately what the issue is and fix it. But, we don’t.
